Management Information Systems Essay Information Systems Failure: The Case of Computer-Aided Dispatch (Cad) System at London Ambulance Service 1. Introduction The LAS covers a geographical area of just over 600 square miles and handles emergencies for a resident population of 6.8 million people. The CAD project is one of the most frequently quoted UK-based examples of information systems failure that took place in early 1990s. The prominence of this particular case is due to the ‘safety critical’ nature of this system and the claim that 20-30 people may have lost their lives as a result of CAD failure. 2. Description of the Manual Dispatch System The manual dispatch system consists of: a) call taking, b) resource identification, and c) resource mobilization. Call Taking: Emergency calls are received by ambulance control. Control assistants write down details of incidents on pre-printed forms. The location of each incident is identified and the reference co-ordinates are recorded on the forms. The forms are then placed on a conveyer belt which transports them to a central collection point. Resource Identification: Other members of ambulance control collect the forms, review the details on the forms and decide which resource allocator should deal with each incident. The resource allocator examines the forms for a particular sector, compares the details against information recorded for each vehicle and decides which resource should be mobilized. The status information on these forms is updated regularly from information received via the radio operator. The resource is recorded on the original form which is then passed on to a dispatcher. Resource mobilization: The dispatcher either telephones the nearest ambulance station or passes instructions to the radio operator if an ambulance is already mobile. A number of problems exist with the manual dispatch system. Most problems are related to the time-consuming and error-prone nature of activities such as: identification of the precise location of an incident, the physical movement of paper forms, and maintaining up-to-date vehicle status information. Therefore, a Computer- Aided Dispatch (CAD) system was considered as a solution to overcome these problems. 3. The Computer-Aided Dispatch System 3.1 Purpose The objective of the CAD system was to automate many of the human-intensive processes involved in the manual despatch system. 3.2 How the CAD system was intended to work The essential features of the CAD system are shown in Figure 1 which illustrates how the system was intended to work in practice. British Telecom (BT) operators would route all 999 calls concerning medical emergencies to LAS headquarters. A total of 18 ‘receivers’ were then expected to record on the system the name, telephone number and address of the caller, and the name, destination address and brief details of the patient. This information would then be transmitted over a LAN to an ‘allocator’. The system would pinpoint the patient’s location on a map. The system was also expected to monitor continuously the location of every ambulance via radio messages transmitted by each vehicle. The system would then determine the nearest ambulance to the patient. Figure 1: The structure of CAD system at LAS Experienced ambulance ‘dispatchers’ were organized into teams based on three zones (south, north-east, and north-west). Dispatchers would be offered details of the three nearest ambulances by the system and the estimated time each would need to reach the scene. The dispatcher would choose an ambulance and send patient details to a small terminal screen located on the dashboard of the ambulance. The ambulance crew would then be expected to confirm that it was on its way. If the selected ambulance was in an ambulance depot then the dispatch message would be received on the station computer. The ambulance crew would always be expected to acknowledge a message. The system would automatically alter HQ of any ambulance where no acknowledgement was made. A follow-up message would then be sent from HQ. The system would detect messages that would tell HQ when the ambulance crew had arrived, when it was on its way to a hospital and when it was free again. 3.3 How the CAD system was built The CAD system was built as an event-based system using a rule-based approach and was intended to interact with a geographical information system (GIS). The system was built by a small software house called Systems Options using their own GIS software (WINGS) running under Microsoft Windows. The GIS communicated with Datatrak’s automatic vehicle tracking system. The system ran on a series of network PCs and file servers supplied by Apricot. 4. Events that Identified the Flaws of the CAD System On the night of the 26th October 1992 (Monday), things started to go wrong at the HQ of LAS. A flood of 999 calls apparently swamped operators’ screens and many of those calls were being wiped off screens for unknown reasons. Claims were later made that 20 to 30 people may have died as a result of ambulance arriving late on the scene. Some ambulances took over three hours to answer a call while the government’s recommended maximum was 17 min utes. Mr. John Wilby, the chief executive officer of LAS resigned within a couple of days of this event. A number of Members of Parliament called for a public inquiry. The Health Secretary initiated an inquiry and the findings were eventually published in an 80-page report in February, 1993, which immediately became headline news in both the computing and the national press. 5. Findings of the Inquiry The inquiry found evidence of poor management practice, high technological complexities and unfavorable operating environment involved in the implementation of the CAD system in LAS. Systems Options, the company responsible for developing the major part of the CAD system had no previous experience of building similar type of systems. This company, which had won the  £1.1 million contract for the development of the CAD system in June 1991, was found to have substantially underbid an established supplier (McDonnellDouglas). Hence, Systems Options was under serious pressure to complete the system quickly. The managing director of a competing software house wrote various memoranda to LAS management in June and July 1991 describing the project as ‘totally and fatally flawed’. It appeared that Mr. Wilby ignored what amounted to an over-ambitious project timetable. Furthermore, an audit report by Anderson Consulting which called for more finance an d longer time scales for the CAD project was suppressed by the project managers. The board of management of LAS was even misled by the project team over the experience of Systems Options and the references supplied by Systems Options were not thoroughly investigated. Due to the extreme time pressure to develop the CAD system within the allocated timeframe, the project team responsible for developing the system practically did not follow any standard systems development approach. As a result, the resultant software was incomplete and unstable. In January 1992, phases one and two of the project began live trials. In March 1992, phase two of the trials was temporarily suspended due to the discovery of system errors. In October 1992, phase three was terminated after two days of reported chaos described above. Questions were raised about the complexity of the technical system. In the manual dispatch system, communication between HQ and ambulances is conducted via telephone or voice radio links. In the CAD system, links between communication, logging and dispatching vi a a GIS were meant to be automated. The automation was completed but no performance testing was thoroughly performed due to the rushed approach to meet the deadline. The system was lightly loaded at start-up on the 26th October, 1992. Any problems, caused by the communications systems (e.g. ambulance crews pressing wrong buttons, or ambulances being in radio black spots) could be effectively managed by staff. However, as the number of ambulance incidents increased, the amount of incorrect vehicle information recorded by the system also increased. This had a knock-on effect in that the system made incorrect allocations on the basis of the information that it had. For example, multiple vehicles were sent to the same incident, or the closet vehicle was not chosen for the dispatch. As a result, the system had fewer ambulance resources to allocate. At the receiving end, patients became frustrated with the delays to ambulances arriving at incidents. This led to an increase in the number of calls made back to the LAS HQ relating to already recorded incidents. The increased volume of calls, together with a slow system and an insufficient number of call-takers, contributed to significant delays in answering the telephones which, in turn, caused further delays to patients. At the ambulance end, crews became increasingly frustrated at incorrect allocations and this led to an increased number of instances where they failed to press the right status buttons. The system therefore appears to have been in a vicious circle of cause and effect. There was also an apparent mismatch of perspectives among LAS management, HQ staff, and ambulance staff. The system has been described as being introduced in an atmosphere of mistrust by staff. There was incomplete ownership of the system by the majority of LAS staff. The hardware and software suppliers involved in this project reported low staff morale and friction between LAS management and workforce. In other words, an atmosphere of hostility towards the computing systems was observed. One of the reasons for low staff morale was that control room staff lacked previous experience of using computer systems. 6. Conclusion In summary, no single element of the case can be regarded as the sole cause for the failure of the CAD system in LAS. The description demonstrates that failure of information systems projects tend to be multi-faceted in nature. Like any hall of fame, its inductees are the best in whatever they do, from baseball or football to something like being President. If you are a member of any hall of fame (including the one for the Presidents), it means that you have done something special or have a certain quality about yourself that makes you worthy to be in a hall of fame. My nominee for the Presidents hall of Fame is our seventh President of the United States, Andrew Jackson. I'll go over his presidency, focusing on both the highs and the lows of his two terms in office, from 1829-1837. The issues that I'll focus on are states' rights, nullification, the tariff, the spoils system, Indian removal and banking policies; these controversies brought forth strong rivalry over his years of president. He was known for his iron will and fiery personality, and strong use of the powers of his office that made his years of presidency to be known as the "Age of Jackson." Andrew Jackson was born on March 15, 1767, in a settlement on the border of North and South Carolina. He was orphaned at age 14. After studying law and becoming a member of the Bar in North Carolina later he moved to Nashville Tennessee. Their he became a member of a powerful political faction led by William Blount. He was married in 1791 to Rachel Donelson Robards, and later remarried to him due to a legal mistake in her prior divorce in 1794. Jackson served as delegate to Tenn. in the 1796 Constitutional convention and a congressman for a year (from 1796-97). He was elected senator in 1797, but financial problems forced him to resign and return to Tennessee in less than a year. Later he served as a Tennessee superior court judge for six years starting in 1798. In 1804 he retired from the bench and moved to Nashville and devoted time to business ventures and his plantation. At this time his political career looked over. In 1814 Jackson was a Major General in the Tennessee Militia, here he was ordered to march against the Creek Indians (who were pro-British in the war of 1812). His goal was achieved at Horseshoe Bend in March of 1814. Eventually he forced All Indians from the area. His victory's impressed some people in Washington and Jackson was put in command of the defense of New Orleans. This show of American strength made Americans feel proud after a war filled with military defeats. Jackson was given the nickname "Old Hickory", and was treated as a national hero. In 1817 he was ordered against the Seminole Indians.

Development of Anna Fitzgerald Character Essay

Adolescence development relies upon many factors. In order to accurately examine its growth, it is useful to look at some developmental theories. Anna Fitzgerald is thirteen years old; however, she is not like any other teenager with some ordinary problems. Anna was born for a specific purpose she was born to save her sister’s life and to serve as a matched tissue donor. When Anna was born, her umbilical cord was collected and since then she was constantly donating blood, stem cells or bone marrow. That resulted in her undergoing more serious and risky procedures. But when she reaches the age 13, she is being told to donate one of her kidneys. Aware of the fact that she was conceived to be a perfect match and ongoing donor for her sister, she wants to have the chance of living her own life. This is when Anna decides to hire a lawyer and to sue her parents to be â€Å"medically emancipated† from her family. Because she loves her sister unconditionally, Anna struggles with her decision. Developmental theories of Piaget, Ericson, Marcia and Freud are very useful, in order to examine the development of Anna Fitzgerald, the character from â€Å"My Sister’s Keeper†. Nature vs. nurture is the first theory that can be applied to Anna’s life. Nature refers to the human biological inheritance and nurture to the environmental experience (Santrock, MacKenzie-Rivers, Malcomson & Leung, 2011). Since she was born for a specific purpose, her parents had already planned her future. To some point of her life, Anna felt it was normal to be a donor and to be in the hospital three to four days a week. Whenever her sister had an emergency, Anna had to be present. The environment Anna lives in is unusual for a teenager. Anna thinks of herself as a total freak. As it is common for teenagers to complain about her look, she states that God must have had some sort of a moody day on her birthday. She sees a big picture of her household. She knows that the environment which she was born in, did not allow her to be a kid. She had to mature fast and act as an adult. It is clear that Anna is going through identity crisis of moratorium. Moratorium stage according to James Marcia is defined by individual exploring different possibilities, yet not being ready to make a commitment to one. In Anna’s case she had plenty of ideas who she would like to be. When asked by her lawyer, where she sees herself in ten years period, she responds: â€Å"There was a time when, like Kate, I’d wanted to be a ballerina. But since then I’ve gone through a thousand different stages: I wanted to be an astronaut. I wanted to be a paleontologist. I wanted to be a backup singer for Aretha Franklin, a member of the Cabinet, a Yellowstone National Park ranger. Now, based on the day, I sometimes want to be a microsurgeon, a poet, a ghost hunter† (Picoult, 2004, p. 412). What strikes the most in her young, yet mature personality is that in ten years period, she would like to be Kate’s sister. Based on Piaget operational stage theory, Anna is clearly capable of using abstract thought. Abstract thought is an adolescence possibility to think outside of the box and see likely outcomes and consequences. Anna knew exactly that by starting the lawsuit, she has a chance of wining the right to decide for her own. Deep inside her, she still wants to help her sister, but knowing the fact that she cannot make her own decisions, made her to go to the extreme and sue her own parents. She is aware of the fact that her decision may have a huge impact on her sister’s life. Perhaps, she will die; however, she is looking at the long term goal. How is the transplant going to affect her life? Is she going to be able to function normally? What if something goes wrong? All this questions were building up inside of her head and did not want to stop. This process of thoughts indicated her ability to think logically by looking at cons and pros of her situation. Based on Ericson psychosocial developmental theory, Anna is going through identity vs. role confusion stage. She is confused of her role in the family. Often reflecting of who she is, and what is the purpose of her life, besides being a perfect match for her sick sister. Anna once said: â€Å"I used to pretend that I was just passing through this family on my way to my real one† (Picoult, 2004, p. 49). This shows how confused she is in terms of her life. Furthermore, this identity confusion grows into her even more upon receiving a long awaited letter of acceptance, into a two weeks hockey summer camp. She is not allowed to go because of her sister’s condition. There is a big chance of Kate going into some health crisis while Anna is gone. It is a difficult time for a thirteen year old girl who is full of energy and is not being able to be just an ordinary adolescence. One can also relate Freud’s theory of development to Anna. The id is one of the structures of human personality. It operates on principles of pleasure and immediate satisfaction regardless of societal rules or other surrounding context (Santrock, MacKenzie-Rivers, Malcomson & Leung, 2011). Anna’s id arises from her frustration to all medical treatments which are done in order to save her older sister, Kate. Frustration is added by her mother who pushes her to donate the kidney for Kate. Yet, from the start of the novel Anna knows the reality which she refuses to face, as result of her inner id: â€Å"On other hand, I was born for specific purpose†¦ I was born because a scientist managed to hook up my mother’s eggs and my father’s sperm to create a specific combination of precious genetic material†¦ specifically, because I could save my sister, Kateâ€Å"(Picoult, 2004, p. 7-8). This inner id, pushes her to rebel against her parents wishes, and results in Anna seeing a lawyer to help her end the suffering and release her from the heavy responsibility towards her sister. This unconscious part of her personality resurfaced, in her reply to the lawyer, when she says: â€Å"Because, she says simply, it never stops† (Picoult, 2004, p. 22). Some may find this very selfish, with total neglect for her sister’s future well being. It results in confrontation with her mother, who tries to make her realize â€Å"You went to a lawyer and made him think is all about you – and it’s not. It’s about us. All of us –â€Å"(Picoult, 2004, p. 54). Thus, her id rises up and does not care if it destroys other people – parents, brother and her sister. In Anna’s case her id prevails over ego. The ego is supposed to negotiate a compromise between her id, current reality and constraints. Anna feels some guilt, as her ego makes her think over and ask herself about her decision regarding kidney. â€Å"I started thinking about this. Would I have to be in the hospital? Would it hurt? Could people live with just one kidney? What if I wound up with kidney failure when I was, like, seventy? Where would I get my spare?†(Picoult, 2004, p.377). Anna’s superego, is supposed to be her moral guide, conscience to do the right thing. It rises up, specifically, when Anna looks at Kate who is becoming weaker and sicker than before and worries about her future and a possibility of her dying. â€Å"What do you think is the best way to die? I don’t want to talk about this, I said. Why? I’m dying. You’re dying. When I frowned, she said, Well, you are. The she grinned. I just happen to be more gifted at it than y ou are†¦ †¦You know, normal people don’t sit around thinking about dying. Liar. Everyone thinks about dying. Everyone thinks about you dying I said. The room went so still†¦ Then a twitchy smile crossed her face. Well, Kate said, at least now you’re telling the truth† (Picoult, 2004, p. 134-135). From this quote it is clear than Anna has difficulty hearing things from Kate, and that her superego is present and possibly regretting the lawsuit action. Perhaps, this is what prompted Anna, to write in her diary that in case of her death, she wants all of her organs to be donated to Kate. In the end, Anna has a car accident and dies, the lawyer who has won the case and got power of attorney, decides to honor Anna’s last wishes: â€Å"I have power of attorney for Anna, he explains, not her parents. And there is a girl upstairs who needs the kidney† (Picoult, 2004, p. 416). Anna’s life ends up tragically. One can assume that she fulfilled her purpos e in her short life, she saved her sister. Since Anna was born, she was a regular donor to her sister. One can observe it as continuity vs. discontinuity development. â€Å"The first time I gave something to my sister, it was a cord blood, and I was a newborn†¦ The next time she relapsed, I was five and I had lymphocytes drawn from me, three times over, because the doctors never seemed to get enough of them the first time around. When that stopped working, they took bone marrow for a transplant. When Kate got infections, I had to donate granulocytes. When she relapsed again, I had to donate peripheral stem cells† (Picoult, 2004, p. 21). One can explain continuity as a process involving a gradual accumulation of behavior or knowledge. Anna, throughout her short life was exposed to medical procedure, terms and responsibilities from the moment of her birth. She was growing up among those circumstances and she never got a chance to be a kid. She had to mature faster. Even her vocabulary was unusual for a thirteen years old girl. In his mind, her lawyer thought â€Å"This girl’s medical vocabulary would put some of my paid experts to shame† (Picoult, 2004, p. 21). Discontinuity is defined as a passing through life stages in a qualitative way. Since Anna’s character is presented just as she is thirteen years old, one can assume that for her to be able to think abstractly, indeed she was at concrete thinking stage in her earlier age. Anna would go through many different stages, perhaps having her case won; she would still donate her kidney. Anna’s life ends abruptly in a car accident. The logical sequence of life is death but to Anna it was way too early. In conclusion, Piaget, Ericson, Marcia and Freud theories were helpful to examine Anna development by using the appropriate key issues. Based on their theories, it is clear to observe Anna’s life and struggles that she is going through. The young age was not an obstacle to deal with some serious adult problems to which Anna was exposed to from an early age. Throughout the story she has dilemmas concerning her sister’s life. Industrial Revolution Essay

Industrial revolution describes the period between 1750 and 1850, in which tremendous changes characterized by developments in textile, iron were realized. The revolution was spearheaded by Britain. Modern historians refer to these changes as the first industrial revolution (Clark, 2007). The second revolution was characterized by steel, electronics and automobiles and was spearheaded by Germany (Clark, 2007). The Industrial Revolution was a period filled with drastic social and economic changes. The transformation between hand-made tools and goods to machine-manufactured products changed not only the economy, but also the lives of the workers. The first changes began in Great Britain in the 1780’s and spread across Europe and North America by the 19th century leaving a profound effect on the entire world. The Industrial Revolution effected every aspect of human society including the nature of work, child labor, and health conditions of the workers. Agriculture was a dominant job for workers before the Industrial Revolution. Sebastian Le Prestre Vauban listed many typical jobs including â€Å"†¦mowing, harvesting, threshing, woodcutting, working the soil and the vineyards, clearing land, ditching, carrying soil to vineyards or elsewhere, laboring for builders and several other tasks†¦Ã¢â‚¬  (Wiesner 152) in his tax-reform proposal. This document shows that life as farmer consisted of purely manual labor. Although these jobs were arduous and demanding, the typical agricultural worker was only employed for half the year according to Vauban. Agriculture was a task-based working system where the work was completed according to a completing a task by a certain deadline. As long as the tasks  were completed on time, the hours spent working were not tightly regimented. With the dawn of the Industrial Revolution, workers moved from the fields to the factories. The Industrial Revolution had a great impact on the human’s rights and conditions and it also resulted in significant technological advancements, but it can be categorically stated that the technological advancements during the Industrial Revolution were paramount when compared to the revolution’s impact upon human rights and conditions. Agriculture was the main-stay for livelihood before the era of Industrial Revolution. Most of the people owned farmlands and workers were employed to work in the farms. Although Industrial revolution brought about significant economic development throughout Europe, there were also considerable social and cultural changes seen as well (Snooks, 2002). Industrial revolution had a tremendous transformation on the middle class, which initially was comprised of industrialists and businessmen to another class of noble and gentry people. There were increased employment opportunities for the ordinary working people in the industries, but under s trict working conditions. There work was monitored and controlled by machines hence long hours of work (Clark, 2007). Industrial revolution led to the introduction of urbanization since many people relocated to the cities to look for employment in factories; such as the water power silk mill and the cotton spinning mill. This was characterized by dense, cramped housing and poor living conditions. There was the introduction of new laws guarding child labor, public health and working condition for the ordinary workers to avoid exploitation of the minority (Snooks, 2002). As much as there were positive effects of the industrial revolution such as urbanization, there was also a negative impact on industrial revolution that comprised of people who were anti technologists such as the luddites (Clark, 2007). There was a change in culture since new cities grew rapidly, affecting families and peer groups. For instance, there was an influence in drugs by peer groups and the following: Economic Changes During the first industrial revolution, there was an unprecedented economic transformation; there was a tremendous increase in population growth that  was sustained. This led to considerable expansions of commercial activities in Europe (Snooks, 2002). Steam power was invented that was used to provide power in the factories, used for mining, and transport. It replaced human labor and introduced machines that could do mining in depth, increase production in the industries, and fast means of transport to the markets. The textile industry was changed by new machines the spinning Jenny allowing for much higher production at lower costs and in less time (Jacob, 1997). Thanks to the industrial revolution that brought about, better transport system such as the canals and then the railway. These provided quick, better means of transporting raw materials from the mines and also finished products to the market. Trade expansion was enabled. There was also much development in metal and chemica l industries due to the industrial revolution that provided better working conditions for its workers (Clark, 2007). Development of all-metal machine tools enabled the manufacture of more production machines for industries. These spread all over Western Europe and North America then to the rest of the world. The industrial revolution facilitated the manufacture of more production machines. Causes of Industrial revolution Industrial revolution came about due to several inventions and the scientific revolution allowing for new discoveries such as technology. Resources required for the industrial revolutions were readily available hence boost industrialization to occur. There was a culture of hard work, developing ideas and risk taking that initiated for the industrial revolution in Europe. Availability of large amount of capital that Europe was ready to use for investment also led to the industrial revolution (Clark, 2007). There was the end of feudalism that changed the economic relationship among the Europe continent, this encouraged industrial revolution. A large population that allowed for industrial workforce was available. Conclusion As much as Western Europe tried to do away with capitalism, industrial revolution contributed to the creation of a true capitalist system. There was wide spread of investments, stock markets, and business corporations. Britain was the main advocator for the industrial revolution due to the agricultural revolution. The British kings lost power and the land holders  gained power (Clark, 2007). There is no doubt that the Industrial Revolution was one of the most influential time periods of human history. It was almost solely responsible for propelling society into the modern economies that we still have in place today. The technological advances of this time are what allowed for the mass production of goods and services for society, which allowed for trade to be conducted on a much larger scale. Additionally, the average family saw in increase in the amount of income that they received because an unskilled worker could find work in one of the many new factories that were opened to pro duce the goods needed for the business world. Unfortunately the workers of the time were usually taken advantage of because of their apparent lack of skill and the abundance of workers available for the same jobs. This created a work environment especially negative for women who would be treated unfairly in the workplace and would receive less pay for the work that they did. Prompted by the oppression that many in the working class felt, literary works were put out to inspire the workers to take back their freedoms. Karl Marx advocated for a revolution of the working class over the management that held them back. Bakunin advocated for the overthrow of the government to get society to a natural state of harmony, and the Pope pushed for a united workers front where the government protected its citizens from being oppressed in the workplace. Over the course of the Industrial Revolution the worker saw vast changes, which ultimately lead to the economic times we have now that are improved from the days of the past. Works Cited Clark, G.
